Students may participate in the School Health Program or have a Private Physical or Dental Examination. 

All Grades need the following:

• Certificate of immunization. Card must be completed & signed by physician or copy of immunization record must be submitted prior to entry into school.

• Emergency Contact Information

• Health History

• HIPPA Form

Each Grade Level then has the following additional requirements:

Grade K (First Entry) or Grade 1:

• Private Physical Form or schedule School Exam

• Private Dental Form or schedule School Exam 

Grade 2:

•  Copy of most recent Private Physical Form and Private Dental Form or schedule School Exams

Grade 3:

•  Private Dental Form or schedule School Exam

• Copy of most recent Private Physical Form or schedule School Exam

Grade 4:

• Copy of most recent Private Physical Form and Private Dental Form or schedule School Exams

Grade 5:

•  Private Dental Form or schedule School Exam 

• Copy of most recent Private Physical Form or schedule School Exam

Grade 6:

• Private Private Physical Form or schedule School Exam

• Copy of most recent Private Dental Form or schedule School Exam 

Grade 7:

• Private Private Dental Form or schedule School Exam

Grades 8, 9, 10
